  • Organized and Federally recognized 11 July 1922 in the Kentucky National Guard at Richmond as the 138th Hospital Company, 113th Medical Regiment, an element of the 38th Division
  • Redesignated 1 January 1937 as Company H, 113th Medical Regiment
  • Inducted into active Federal service 17 January 1941 at Richmond
  • Disbanded 10 February 1942
  • Reconstituted 25 August 1945
  • Consolidated 18 March 1947 with the Clearing Company, 113th Medical Battalion (see ANNEX 1), and consolidated unit organized and Federally recognized in the Kentucky National Guard at Richmond as Battery C, 441st Field Artillery Battalion; concurrently, relieved from assignment to the 38th Infantry Division
  • Consolidated 1 October 1959 with Headquarters Battery, 441st Field Artillery Battalion (see ANNEX 2), and consolidated unit designated as Headquarters Battery, 3d Howitzer Battalion, 138th Artillery
  • Consolidated 1 March 1968 with Battery C, 3d Howitzer Battalion, 138th Artillery (organized and Federally recognized 7 December 1965 at Richmond), and consolidated unit designated as Headquarters and Headquarters Company, 103d Supply and Service Battalion
  • Reorganized and redesignated 1 November 1980 as Headquarters and Headquarters Company, 103d Support Battalion
  • Consolidated 1 November 1985 with Detachment 1, Company C, 1st Battalion, 149th Infantry (organized and Federally recognized 1 May 1974 at Richmond), and consolidated unit designated as Company B, 4th Support Battalion, 35th Infantry Division Support Command
  • Reorganized and redesignated 1 June 1987 as Company B, 735th Support Battalion
  • Converted and redesignated 1 December 1989 as Detachment 1, 2123d Transportation Company, and relieved from assignment to the 35th Infantry Division
  • Ordered into active Federal service 11 October 1990 at Richmond; released from active Federal service 11 June 1991 and reverted to state control
  • Reorganized and redesignated 1 October 2001 as the 2123d Transportation Company
  • Ordered into active Federal service 7 December 2003 at Richmond; released from active Federal service 3 June 2005 and reverted to state control
  • Ordered into active Federal service 2 February 2010 at Richmond; released from active Federal service 8 March 2011 and reverted to state control
