- Reorganized and redesignated 16 November 1993 as Headquarters and Headquarters Company, 4th Battalion, 4th Aviation (organic elements concurrently constituted and activated)
- Battalion inactivated 15 February 1996 at Fort Carson, Colorado, and relieved from assignment to the 4th Infantry Division
- Assigned 16 May 2005 to the Combat Aviation Brigade, 4th Infantry Division, and activated at Fort Hood, Texas
- Redesignated 1 October 2005 as 4th Battalion, 4th Aviation Regiment
- Inactivated 15 September 2011 at Fort Hood, Texas, and relieved from assignment to the Combat Aviation Brigade, 4th Infantry Division
- Assigned 16 April 2014 to the Combat Aviation Brigade, 4th Infantry Division, and activated at Fort Carson, Colorado
