Organized in October 1917 in the National Army at Seattle, Washington, as Base Hospital No. 50

Demobilized 5 May 1919 at Camp Lewis, Washington

Reconstituted 19 October 1936 in the Organized Reserves; concurrently consolidated with the 50th General Hospital (constituted in July 1923 in the Organized Reserves) and consolidated unit designated as the 50th General Hospital

Organized in 1937 at Chicago, Illinois

Ordered into active military service 4 September 1942 and reorganized at Camp Carson, Colorado

Inactivated 27 October 1945 at Camp Kilmer, New Jersey

(Organized Reserves redesignated 25 March 1948 as the Organized Reserve Corps; redesignated 9 July 1952 as the Army Reserve)

Activated 15 July 1948 at Seattle, Washington

Ordered into active military service 21 November 1990 at Seattle, Washington; released from active military service 21 March 1991 and reverted to reserve status
