Demobilized 14 March 1919 at Camp Upton, New York

Reconstituted 9 November 1936 in the Regular Army and concurrently consolidated with the 5th Evacuation Hospital (constituted 17 November 1923 in the Organized Reserves as Evacuation Hospital No. 5; redesignated 23 March 1925 as the 5th Evacuation Hospital; withdrawn 1 October 1933 from the Organized Reserves and allotted to the Regular Army) and consolidated unit designated as the 5th Evacuation Hospital

Activated 15 June 1942 at Camp Rucker, Alabama

Reorganized and redesignated 12 March 1943 as the 5th Evacuation Hospital, Semimobile

Inactivated 5 March 1946 at Camp Kilmer, New Jersey

Activated 2 October 1950 at Fort Bragg, North Carolina

Reorganized and redesignated 12 June 1953 as the 5th Evacuation Hospital

Reorganized and redesignated 26 June 1972 as the 5th Combat Support Hospital

Reorganized and redesignated 16 April 1982 as the 5th Surgical Hospital
